Pre-Entry Health Requirements COMPLETE BY JULY 6 JMU policy and state law requires students to submit specific information about immunizations and complete a TB Assessment Form. Immunizations must be up to date. Any official immunization record will be accepted, but a physical exam is not required. You must complete the following: 1. Log onto MyJMUChart Patient Portal and review the set of instructions for new students including what tabs to click on and what to upload. q Check the status of your immunizations q Complete the TB Assessment Form (6 questions) q Upload your insurance information 2. Check your JMU email and secure messages on MyJMUChart Patient Portal for confirmation that your record has been verified and is complete. Your pre-entry health requirements are not complete until you receive that confirmation. An enrollment hold impacting your ability to enroll in future semester classes and a fine will be placed on your account if your immunizations and TB Assessment Form are not deemed complete by the Health Center staff.
University Health Center (UHC) The UHC is a confidential acute care clinic and is located in the Student Success Center. You can select whether you would like the UHC to share information from each visit. As you prepare, confirm that current insurance will cover you while at JMU for specialists or emergency care. A Medical Power of Attorney is not needed.
As you transition to self-managed health care, start now by managing any oral medications that you currently take. Consider getting a 90-day supply of any oral medications or using a mail order option. You can also check out the on-campus pharmacy or locate the closest local pharmacy for prescription refills or your over-the-counter needs.
