State law requires students to submit specific information about immunizations and complete a TB assess- ment form (six questions). Immunizations must be up to date. Any official immunization record will be accepted. JMU does not require a physical exam, but you must complete the following: 1. Log onto MyJMUChart Patient Portal to check the status of your immunizations, complete the TB Assess- ment and upload your insurance information. 2. Continue checking your JMU email and secure messages on MyJMUChart Patient Portal for confirmation that your record has been verified and is complete. Your pre-entry health requirements are not complete until you receive that confirmation. An enrollment hold and fine will be placed on your account if your immuniza- tions and TB Assessment are not deemed complete by the Health Center staff.

Be sure to send your final high school transcript with final grades and graduation date to JMU Admissions as soon as pos- sible. If you have dual enrollment, AP/IB test scores or any other courses you hope to receive credit for, you should submit an offi- cial college transcript and/or test scores to JMU Admissions for credit evaluation. Refer to these helpful resources for dual enroll- ment credit and test credit.

Tips for transiting to self-managed health care ◆ UHC is a confidential resource. ◆ Students can select whether to share information from each visit and with who. ◆ A Medical Power of Attorney is not needed for UHC ◆ Students are responsible for managing their oral medications. ◆ Confirm that current insurance will cover you while at JMU for specialists or emergency care. ◆ Check out the on-campus pharmacy or locate the closest local pharmacy. ◆ Consider getting a 90-day supply of any oral medications or using a mail order option.
